Investigating Mechanistic PRedictors Of interpatient Variability and temozolomide (TMZ) induced haematological toxicity for glioma patients.
IMPROVE TMZ is a translational study that is an investigator initiated trial. This study is for glioma patients who are being started on temozolomide treatment or who have been treated with temozolide (TMZ) and developed toxicity (wording taken from ucc cancertrials site). The development of severe TMZ-induced haematological toxicity is due to higher exposure to TMZ in plasma, driven by mechanistic factors, such as pharmacogenomic variants, the microbiome, or demographic factors. The primary aim of the study is to determine potential predisposition factors for severe haematological toxicity from TMZ.
The study has been designed to incorporate 2 parts. Part A cohort will study patients who will receive or plan to receive TMZ for high grade glioma. Biospecimen samples (blood, stool and DNA) will be tested to assess for onset of haematological toxicity. Part B cohort will study patients who are receiving treatment with TMZ for high grade glioma and have developed haematological toxicity associated with TMZ.
These results will help to understand why some people treated with the chemotherapy drug temozolomide develop serious blood related side effects while others do not. It will look at how the body processes the drug, whether gut bacteria and genetic differences influence drug levels and side effects, and whether simple cheek swab tests can reflect information usually taken from tumour samples. The study will also examine how these side effects affect patients’ well being, stress levels, and quality of life during treatment.
